Population Health Essentials and Population Health Management and Strategy provide a comprehensive overview of the tenets of population health, including the key components of creating a population health model/framework.

Combined, these programs provide a solid foundation for those seeking to advance their understanding of population health and to gain the skills needed to develop a population health management strategy. Register for one or both of these programs!

POPULATION HEALTH ESSENTIALS

A 3-day comprehensive overview of population health this session outlines the fundamentals necessary to improve outcomes and reduce costs.

Topics include: value-based care, social determinants of health, quality and safety, health economics, population health intelligence and an overview of population health management.

Participants in this program are encouraged to attend Population Health Management and Strategy.

Who Should Attend: Healthcare professionals, leaders and stakeholders from across the industry

POPULATION HEALTH MANAGEMENT
AND STRATEGY

A 2-day workshop that builds upon concepts learned in Population Health Essentials. The focus is on developing actionable strategies and practical solutions to implement population health management programs that improve clinical and financial outcomes.

Who Should Attend: Healthcare professionals, especially clinicians and practitioners with an understanding of population health principles. Graduates of previous Pop Health Academy programs including those who are enrolled in or have completed Population Health Essentials.

HIGH RELIABILITY BOOT CAMP (Oct 21-22)

Optimizing care quality is an essential component of success in today's value-based payment environment. The High Reliability Boot Camp is an intensive 2-day interactive program that will provide the tools, framework and, strategies needed for high reliability in both the inpatient and outpatient setting.

Day 1 of the program will review basic concepts of high reliability, including communications, structural organization and culture. Day 2 will focus on learning to integrates key process improvement strategies to build a culture reliability, safety, quality and service excellence. The program includes post Boot Camp coaching calls.

Who should attend: Healthcare professionals, especially those with responsibilities in the areas of clinical delivery, finance, information services, imaging, pharmacy, and /or quality improvement
