Primary And Specialty Integration To Improve Cardiovascular Care

Incorporating specialty care into value-based care models has lagged overall, and a critical case in point is cardiovascular care. This lack of integration contributes to the overutilization of unnecessary cardiac care, including stenting and diagnostic testing, which has significant clinical and financial implications. In this breakout, One Medical and Heartbeat Health will discuss how they drive high-value care through their integrated, collaborative care cardiology model. Clinicians from Optum Health and WellMed Medical Group will discuss how they utilize a coronary computed tomography angiography (CCTA)-first approach to patients with stable chest pain in primary care settings.

Assuming Global Risk For Pharmaceuticals

Assuming global risk for the total costs of care is a goal for many APG member organizations, but relatively few assume risk for pharmaceuticals along with other aspects of care. In this session, speakers from organizations that have assumed global pharmaceutical risk—or have assisted others in managing it—will discuss opportunities and challenges of the approach. They will describe how assuming global pharmaceutical risk can enable providers to better manage costs and improve patient outcomes, as well as the capabilities needed to take on and manage these risks. They will also discuss how-to's of assuming global pharmaceutical risk, including making financial projections of costs and benefits.
