SAN DIEGO, CA USA -- HEALTHCARE UPDATE NEWS SERVICE™ -- SEPTEMBER 1, 2026: CMS's ACCESS Model went live in July 2026, bringing outcome-aligned payment for technology-supported chronic care into Original Medicare for the next ten years. For ACOs, the most consequential detail is one of timing: ACCESS expenditures stay outside Medicare Shared Savings Program and ACO REACH benchmark and performance year calculations through 2027, and are included beginning in 2028. Examine what that window makes possible in this webinar presented by three ACCESS participants, each operating in a different clinical track.

Examine the mechanics most ACOs have not yet worked through:

  • How beneficiaries align to participants, and the three routes into the model: clinician referral, patient self-enrolment, and claims-based identification
  • Outcome-aligned payment across the four clinical tracks, and how outcome attainment thresholds determine what participants are paid
  • Co-management payment mechanics, including which practitioners are eligible to bill, the documentation standard, and delegation to auxiliary personnel under general supervision
  • How enrollment stacks when a beneficiary is aligned across multiple tracks at the same time
  • Benchmark and performance year treatment through 2027, and what changes in 2028
As CMS moves toward outcome-based payment for chronic care at national scale, the next two model years give ACOs a period to build their own outcomes and utilization data before those expenditures affect benchmark calculations. This webinar is directed toward ACO and Health System Leaders, Medical Directors, Value-Based Care and Finance Leadership, and Practice Operations teams.

Join us to hear about ACCESS from the actual participants who know the details inside and out!
